Overview

Anonychia congenita is a rare genetic condition where a baby is born without all or some of their fingernails and toenails. 

Symptoms

Missing nails: Complete or partial absence (hyponychia) of nail plates on fingers and toes from birth.

Normal surrounding skin: The skin, tips of digits, and nail beds look and function normally.

No systemic illness: The condition is typically painless and does not cause other major health problems

Bone defects: Missing or underdeveloped finger bones, toe bones, or kneecaps.Teeth abnormalities: Unusual shape or development of teeth.Hair or skin changes: Sparse hair or unusual skin pigmentation patterns.

Causes

RSPO4 Gene Mutations: Mutations in the RSPO4 (R-spondin 4) gene on chromosome 20p13 are the most common cause. This gene helps control the Wnt signaling pathway, which is essential for forming tissues like fingernails and toenails.naffected carriers. Autosomal Dominant Inheritance: Less commonly, some families show an autosomal dominant pattern, meaning a single copy of the mutated gene from one parent can cause the condition. 

Other Gene Mutations: Changes in other developmental genes, such as PADI3, can also contribute to congenital nail absence.

Associated Syndromes: Anonychia can happen as part of genetic syndromes like DOOR syndrome, nail-patella syndrome, or Coffin-Siris syndrome. These syndromes involve other developmental abnormalities alongside nail loss. 

Teratogenic Exposure: Rare cases occur due to exposure to harmful substances or medications during pregnancy, such as the anti-seizure medication phenytoin or the blood thinner warfarin. 

Diagnosis

Physical Examination: A doctor checks the fingers and toes to see if fingernails and toenails are completely or partially missing (hyponychia), while confirming that surrounding finger and toe tissues are otherwise normal. 

Exclusion of Syndromes: Physicians perform a broad assessment to rule out other health conditions or syndromes (such as nail-patella syndrome or ectoderm congenital-anonychia-a-distinctive-nail-finding-in-a-newborn)X-ray Imaging: X-rays of the hands and feet may be used to verify that the underlying bone structures and distal phalanges (fingertip bones) are normal and not underdeveloped.congenital-anonychia-a-distinctive-nail-finding-in-a-newborn)

Genetic Testing: Molecular genetic testing can confirm the diagnosis by identifying mutations in the RSPO4 (R-spondin 4) gene located on chromosome 20p13

Treatment

Observation (Masterly Inactivity): For isolated cases without pain or functional impairment, no specific medical or surgical therapy is necessary.

Protection and Hygiene: Keeping the exposed nail beds clean and using protective gloves or footwear helps prevent accidental trauma to the sensitive tips of the fingers and toes.

Psychological Support: Counseling or peer support can assist children or adolescents dealing with self-esteem challenges, teasing, or body-image stigma.
